Wheel bearings seized due to no oil would be my first guess. Driver might not even notice. Seen a couple of those smoky trails on the highway for many miles.
Or a brake pod with s-cam stuck in the ON position.
Or someone didn't make replace the frozen front shoe roller with the grove worn into it. S-cam over-rotated and got stuck.
