If there's no placard they need to be reported. But I've heard of folks getting harassed when they have a placard because they don't "look" handicapped. I remember reading a story 35 years ago where a guy in his mid 20s was frequently harassed despite having a placard because he was in his 20s and appeared very fit, but he had muscular dystrophy. He spent a lot of time in the gym to maintain his fitness/strength as much as possible but fatigued very quickly if he had to walk from the back of a parking lot.
Still, there are lots of times that I see someone with a placard and think it's BS. But I don't say anything because you never know what's really going on. I'm sure that there are dirtbags that have a placard that are gaming the system. I'm also sure that there are folks that have a placard because they "take care of granny" but probably use it when granny isn't with them. But there's always going to be those folks, and you can't tell who is and isn't by appearances. It's better to assume that anyone with a placard is legit.
If there's no placard or plate, then they need a sticker or several.
