Japanese Space One Kairos Rocket Unscheduled Rapid Dissassembly
I extracted this frame from a news article video. This is near the exact moment when the failure began. Looks to me like the second stage ignited prematurely, but I'm not a Rocket Scientist, so we'll wait on the failure report.
