If you plug your phone into the computer, it will show up as a hard drive. The photos are in a totally random order, and it makes no sense.
I ended up building folders on a hard drive of my computer. If you have those old folders on your phone, just use iTunes to show it where photos are stored, and select them to be backed up and restored to your phone.
If you add a new folder, you will need iTunes to select that folder to restore and back up. Just put all the folders in a folder named something like Myphotos.
In the end, by working around the useless iTunes, it can be done. Just do a full backup of your phone to your computer, then add the folder to the Photos section of iTunes so it will back up and restore from there.

If your computer is a MAC, it might be easier. It really seems like Apple made it hard for Windows users.
