At what point do you stop fighting and suffering and just agree to "Shuffle off this mortal coil" in peace?

I watched a friend suffer with cancer and watched my father suffer with dementia. I'm pretty sure I would want to avoid that, but I've never been in that position.
My uncle passed from cancer (started in his throat, I believe) a few years ago. They think it was from agent orange. He had it and went through all of the treatments. He had a very high pain tolerance which is apparently a good thing. It cleared up and I think he got a year or so, and then it came back. His family REALLY wanted him to fight and go through all of the possible treatments, but that second time, he told them "no, I'm not going through any treatment." My mom (his sister) was with him very near the end. She got to speak to him without other family around. She asked him "does it hurt?" He said, "the cancer, no, not at all." He then told her that the treatment is painful. He passed fairly quickly, thankfully, although his family (some of them at least) were not happy that he didn't go through another round of treatment.
