If your 4006 lost it's config then you need to open a case with the TAC. There's definitely something wrong. On the 4006 you are running the pre-IOS style code, aren't you? Or have they moved those to the IOS command line as well? I haven't worked with one in quite a while. We have 29xx/35xx and 6500s.
When a switch boots up with no config then every port is automatically on vlan 1. It's Cisco's version of plug and play, you can take a switch out of the box and it'll switch packets. So if your ports aren't doing that then there's something else to look into.
I'm sure you've got it configured by now, just giving you some stuff to think about.
