First off, I'm sad to see this happen to Sen. Johnson. IMO, we need more politicians like him. If the Democratic Party was made up of politicians like him, I would vote Democrat more often. Unfortunately, the shrillness of the Kennedy's, Boxers, and Pelosis seem to dominate.

That being said, I'm going to assume he is replaced and a Republican is appointed. What does this mean for the Senate and Congress as a whole?

First off, Pelosi is out.
Er, no, Pelosi is in the House of Representatives, not the Senate. Her Speakership isn't in jeapardy as a result of Senator Johnson's illness.

It was interesting to see her immediately start to talk down expectations for her tenure, but now I would expect the blame game to pick up right where it left off. Too bad, as I was somewhat looking forward to seeing them falling flat on their faces. Watching them blame Republicans for their screw-ups while they were in power was going to be fun. (Of course the AP, CNN, MSNBC, the broadcast netoworks, Time, Newsweeks, the New York Times, The LA Times, the Chicago Tribune, and NPR to name a few would endorse this point of view.)
Again, Rep. Nancy Pelosi, D-CA isn't going to lose her position as Speaker of the House anytime before 2008, barring any unexpected events in the House.
