This is how Howard Dean did it in Vermont, it's a user/payer insurance plan not a Canadian style system at all.
Two or three states already have their own "health insurance plans", and as I understand it these could still exist. You could always opp-out and pay another plan/company of your choice in Vermont where the state acts more as a regulator than an administrator.
