I'm miles from being an expert on etiquette
but removing gloves for a hand shake seems like the common sense respectful thing to do.
Only time I would decline a handshake is if my hands are really dirty in which case I will show the palms of my hands and say "sorry".
And how about the opposite of the cold fish handshake... the guy who tries to rip your arm off? It's a social greeting moron not a test of strength!
