well, that's not entirely true Joe,
can't speak for all of Europe, but in Belgium, Netherlands,Germany, UK, and i believe France as well, for a citizen there is no requirement that forces high incomes to get private insurance...
you can however, get extra insurance, for stuff like a private room in a hospital instead of a shared room
