I would normally give a "Thank you" at the first and follow up with "Thanks again" or just "...and again" and a smile at the second.
My Dad will sometimes thank a guy holding a door open with;
"You're a Gentlemen - not many of us left"
I like Tabs' opinion. But I don't think people are deliberately being rude - they're just ignorant.
