Do you know that the relay is firing? Disconnect the horns, push the horn button, and listen for the relay clicking. If it's working and wired correctly, you've got issues with either the horn button switch or the rubber collar. If it doesn't click, try disconnecting the negative battery cable and listening for a click. If the horn button switch is broken the relay might be powered all the time, thus, the horns will always be on.

Does the horn honk continuously with the horn pad off the wheel? With the horn switch pulled out of the wheel? Does the switch move in and out freely? Is the rubber collar on the back of the horn pad deformed or dried out and cracked? That will cause the horn pad to sit crooked on the wheel and could push the switch in.
