Odds are he had multiple doctors prescribing the stuff and they did not know about the other docs. Happens all the time. It is one reason that some people want centralized medical records. It doesn't just happen with "fun" drugs...you can get toxic interactions from damn near anything.
If the patient doesn't disclose to the doctor what meds he/she is on, how can you hold the doc responsible?
I've mixed and matched in my past. Maybe I was lucky or just had a better understanding of what was going on. Mistakes happen though...I know that one too.
