This actually isn't true. The kid can fully understand that not every action gets the reward. For example, my son was deathly afraid to go to the dentist. We offered him a special toy as a reward if he went to the dentist and didn't cry. It worked. He went last week and asked for a reward and we simply explained that he doesn't get a reward anymore since he can do it now and is a big boy. He accepted that just fine.
