The fast idle pretty much has to be a vacuum leak. Advanced timing can contribute.

The bucking and dying when hot makes me wonder about the head temp sensor. Sounds like something is suddenly making it run WAY rich. Is it just bad at idle, or can you then rev it and smooth it out? If so I would say another vote towards rich fuel rather than ignition.
