There was a head-on EL wreck in Chicago a few months ago.
Investigation found that the operator stopped the train at a red light, got out and manually switched the switch and overrode two automatic kill mechanisms. Sometimes it's not a switching error.
The operator was placed on paid leave (gotta love the unions).
