I was heading around 45mph and I heard a metallic CLANK and then I lost all of my power steering. I stopped off at the nearest parking lot and looked underneath to find that the back mounting bolt and both sleeves were missing! The belt was still hanging from the pulley however.

This is probably related to the fact that my power steering pulley off the crankbolt is not centered properly, or broken near its center, causing the spin to be off-center.
1.) What would cause my crank pulley (944.102.214.03) to wobble off centre during rotation?
2.) Would this most likely cause the uneven stress on the bolts/belts/sleeves of the power steering pump, ultimately causing them to jump ship?
3.) The new power steering mounting bolt is 8x38mm. Is this something standard I can get locally?
