They secrete (sp) some stuff, so can get blocked. My cat had one that got what looked like a blister and the vet was concerned about anesthetic as she was 21 years old so didn't want to operate on her. But Chalkie being Chalkie, she just chewed it off and was fine. Cats are in some ways self maintaining. If you can keep them away from the vet nature is best in some situations.
