I've done quite a bit of interstate driving in the U.S.A. and try to keep it smooth whether I'm going slow or fast. That means anticipating potential bottle-necks before they occur and definitely never pulling out in front of someone going fast in the left lane. If you are going faster, even if you have a convoy close behind you it will not be long before you're past. Think about it; if the convoy is hauling ass in tight formation, they will blow past. If they're not tight and fast, there will be a big hole to jump into. The worst is when you're driving something with no passing power like a loaded truck/trailer, then you really need to plan your moves in advance to maintain speed.

Big rigs are a good example usually on American highways in terms of passing etiquette and lane discipline. They do not generally park their asses in the left lane or make clumsy moves. (There are exceptions of course). It still pisses me off to no end when someone just parks in the left lane with their cruise control on. It doesn't matter how fast or slow they are going, they are preventing people from using the lane for passing. Plus it makes us look like stupid/lazy/selfish Americans to visitors from other more civilised places.

Whether in town or on a highway, good drivers have good natural rhythm and it's a joy to share the road with a bunch of them. The opposite is also true.
