If the article was correct the first two were at the original stop. If someone really wants to shoot an officer they can do it, though I'm not sure how he got both of them. I thought that SOP was to have one approach while the other stayed back.
As for the entry, one article said he was hiding in a closet and shot from there. In that case, just bad luck for the officers. Not sure what they can do about that...you enter a room to clear it and there is a closed closet. You can't just put a few rounds in there "just in case."
