Having personally flown for a UPS feeder and dealt with their operation, I can say they actually are quite careful with their cargo. Yes, things happen but overall their folks are trained to be reasonably careful with packages, to screen (six-sided-checks) packages for any hazmat warning labels, etc. There are multiple layers of checks and double checks on these things and although mistakes do sometimes happen, they're not too common.
My suspicion is that the box with that muffler on it somehow got misplaced or wedged under a truck and a driver took off, dragged it a short distance, etc. before someone either flagged him down to get his attention or stopped himself to check it out. Just a guess, but having observed how they work, that's my guess. Most of their guys are good, but UPS really does beat on their drivers hard to keep schedules - that's when mistakes can happen (when people get in a rush).
