Congrats! Wish I saw your post 2 days ago - I don't think they actually require the RIV form for a trip permit. What you need is a "special" permit (they changed it all last July). Here's the MTO website:

What you need for a US car is the permit form, original signed title and insurance policy info. It says nothing about the RIV form.
