The entire front face of the engine would be gears to get the rotation up there. It would look like a clock. Think about the weight and noise of all those gears. Then there is the heat generation that has got to go somewhere. Then there is the motor oil to lube it. Along with that is a bunch of expense.
They will stick with belts and chains - or pushrods.
