They are designed to be a traction device, rather than simply a blood pressure inverter.

they take up a lot of space, and are very heavy-duty. While there are plenty of other ways to decompress one's skeleton, that piece of kit does make it easy.

IMO, they don't help with muscle imbalance - which seems to be the root of many back problems. YMMV.
