I think what sets people off is when other drivers do not behave in a predictable manner. Meaning there are behaviors that we come to expect/anticipate as a norm. Those behaviors may not always conform to the law but they are what most of us have "agreed" to.

Example is a one lane with a solid white line on the right side. If a vehicle stops to make a left turn off that road we go around it on the right shoulder. Technicaly not legal but we do it and when someone stops behind turning vehicle it breaks the flow and is unexpected behavior.
