Is it possible that your fan pulley half has worn into the the mating portion of the fan. I have seen a groove worn into the fan side that then allows the pulley half to slide into it. It then becomes off-center and appears to be a loose fan belt, with the associated belt vibration, etc.
This is usually the result of over tightening the belts.
