Keep it small (but large enough to be profitable).
Hire smartly. It is amazing how much I prefer to go to places with a beautiful wait staff.
Know your audience. If you are near a university, best to keep sell reasonably priced beers, not primium stuff.
Good grub at a reasonable price will keep people coming back.
