By SoCal standards the drive from Hilo to Kona is no big deal. I've driven around the island multiple times. I still think it can be tough for a haole on any of the islands, but probably less of an issue if you're not raising kids. As for island fever, depends on what you want to do. I have a fantasy of going there and playing music for the tourists by night, lounging by day after I retire. Would probably get old after awhile though, and from a music standpoint if you don't dig the "island sound" you'll kinda go nuts.
