What I did find is that the stop nut was missing and the adjustment bolt had backed off pretty far. The other side was worse. It had backed off far enough that it fell off and was lost some time ago. He wasn't kidding how easy it was.

Here's the cause. Not a bad idea to check the lock nuts any time your wheels are off. I just got the caps for the other side but now I'm waiting on the adjusting bolt to come in!
