Are you sure the gear is inside. Was there a cap on the adjuster before you started the brake job? It may have worked loose. It can sometimes pull out when you retrack the hex key. I know it's tough to see but get a flashlight and a mirror and try to inspect the adjuster from the inside of the control arm access port.

-another possibility is that when you unscrewed it to fit the pad you unscrewed it too much and the thread disconnected from the piston. I did the same thing after rebuilding my first set of rear calipers. Now that was fun.
