My Starion had calipers like that, hated them.
If the face of the piston has notches the pad may have alignment nubs.
1st time you pump the brakes to set the pad the piston can turn a bit and the nubs end up contacting the piston face and not falling in the notches.
Happened to me once.
Can cause what you describe.
