Everyone has different expectations for what something should look like when finished. Did you see his work somewhere else that you can reference? Are they the same or similar?

Adding 3" of concrete on top is going to be a solution that will fail at some point or another. It should be 4". If it was added, then you'll need to transition the grade to the concrete, which means adding dirt and maybe sod, etc.
