My clutch failed in one normal shift with absolutely no prior warning. Was working perfectly fine and then pedal went straight to the floor and stayed there. Once it starts slipping, its gone.

Clutch plate was in several pieces and bearing was shot. Stock dual mass flywheel worn out also.

Check the slave cylinder first just to be sure, but if its ok then its time to pull the trans and inspect the clutch, bearing, flywheel (be sure to test it), etc.
