The white plastic piece slips over the pin on the base of the lever and is held on by a washer and thin retaining clip (the diagram and the top of thread shows this well). These were a pain for me to get on.

The other end of the plastic piece is forked. The cable fits between the forked end which sits behind the plunger/adjustment sleeve. When you pull the cold start throttle lever up, the white plastic piece pushes the plunger and cable which depresses the gas pedal and sets the cold start valve in the engine. The open end of the fork should be facing the drivers door. Make sure that the adjustment sleeve screw is visible through the small hole as pictued in RoninB's post when you are done. (You don't need to touch this when putting the plastic piece in, but it could move in the process of replacement.) This enables you to make any adjustments to idle setting on cold starts going forward.
