Herschel worked on him there at the house, and I see what you mean, but we do not know how much
story time transpired between last week's episode and this week's... it may have been a couple of weeks... or more.
Also, remember he had his legs tied (ankles?) and I can't remember if we saw him walking with a limp or not after he cut the rope. I'm sure he's not 100%.
