Yes!
Short (just over 100 yards) second hole at the Radium Resort in BC.
Was with my ex father in law when it happened. Funny thing is that the green is slightly elevated from the tee box and I didn't get to see it drop. I could hear the people on the number 3 tee box cheering though as it approached and fell into the hole.
Screwed up the rest of my round!
