A buddy of mine got a monster ticket for parking in a HC spot with no sign in front of it and what he later recognized as a very faded old HC logo on the asphalt. This was 20 yrs. ago. He went back with a camera the next day and they had repainted it and put a sign in front. I've seen plenty of parking lots that were resurfaced or redesigned with HC spots moved and the old ones not covered or paint removed, making it sort of iffy as to whether it still was a HC spot.
I got a parking ticket once for parking in front of a bare pole where a no parking sign had once been. Cop sitting in his car watched me do it, said nothing and had put a ticket on my windshield by the time I came out 10 min. later. I called him up and bent his ear about it. He said he'd see me in court. Judge took one look at the cop's photo, didn't even ask for mine and dismissed it all. I walked over to the cop and thanked him for coming to court.
