Ask a dog person outside the family over to watch her move about and see what they say.
A friend of a friend had a small terrier that was very old, blind and deaf. It made me cringe to look at her (the dog, not the friend...).
The loss of control of her bodily functions would be pretty much it for me but I understand where you are coming from. I still remember the moment I put down our family dog when I was 19 and the dog was 15. One of the hardest things I have ever done.
