I had that too but on a cheap worm gear clamp, not OEM.
2nd situation is the bellcrank behind gas pedal. Bushings worn out and bellcrank swayed to side catching under sheetmetal on floorboard. 1st time happened, was doing 118MPH on a track. Let off gas and was still pulling strong! Hit the key off....
