I believe you and Panzer have it figured out with the throwout bearing. If it's not, you'll probably find out what it is (rubber going, fork assy bad, pilot bearing, etc) when you check the throw out bearing.

Is your timing really off, maybe from the set screw on flywheel in the wrong place, or is it running rough from not as much dampening?

edit: noise may be the torque tube since it quits when clutch is mashed.
