Actually, around here 3" is uncommon because it's smaller than what most of the diesels are running.

A friend of mine runs a muffler shop. He made the custom dual exhaust for my El Camino. While he was welding it up we chatted. He said about 3 times a year some pickup owner comes in and wants dual stacks coming up behind the cab area. He explains he will have to cut holes in the bed for the pipe and once the hole is cut there is no going back. They still want it done.

Lots and lots of trucks around here have the semi style stacks. I've seen several that were run through the bed the way you describe, but I've also seen a few where the bed has been moved back far enough to allow the exhaust to be run between the bed and cab.

It's not uncommon around here to see exhaust on diesel trucks between 4 and 8".
