Actually, around here 3" is uncommon because it's smaller than what most of the diesels are running.

Lots and lots of trucks around here have the semi style stacks. I've seen several that were run through the bed the way you describe, but I've also seen a few where the bed has been moved back far enough to allow the exhaust to be run between the bed and cab.

It's not uncommon around here to see exhaust on diesel trucks between 4 and 8".
