I talked to a doctor once about what it was like working those long hours for average money. (I was a nurse at the time)
He said it was a "Calling" and there's nothing else he wanted to do with his life.

He also said that "Calling" is what keeps you working those 14-16 hour shifts.

So for some, maybe a lot there is no choice.
