These are the difficult ones.
It the bottle is neck down, you can push a plaster of Paris impregnated bandage (such as used to be used for fractures) into the opening, wait for it to set, and pull the bottle out.
If the bottom of the bottle is facing down, then it usually means a laparotomy. Although I did once see a (slightly built,) fatigued, surgical registrar near the end of a 72 hour shift (and the end of their tether) say "F#$% this" and disappear almost up to their elbow to retrieve a glass Coca-cola "buddy" bottle the same way it went in. (Yes, the patient was anaesthetised at the time, but it avoided a 2 am laparotomy that no-one wanted to do).
