There are two ways:
1: Use the open end of a wrench and hit up using a mallet at the base. If your VERY lucky it will come off.

If that doesn't work, and it probably won't due to age, you can try again with heat. I used a heat gun and mine still did not budge.

Failing all else you can try method two: CAREFULLY use a dremel to cut into the plastic at the base. You need to go extra slow here as you can mar the metal handle underneath.

Both methods will result in the destruction of your old shiftknob. If you want to keep it for "originality", I recommend you find a used handle with out a shiftknob attached and use that one.
