One has to assume that Bill Cosby, at the height of his popularity and earning power in the 1980s, had a hundred attractive women trying to get themselves impregnated by him. Standard celebrity or wealth/power situation. It's a "he said, she said" and thus very difficult, if not impossible to prove in actual court. The court of public opinion might yield some money so she's on TV now. Settlements occur in order to make the problem go away and do not imply guilt on either party.
One might also assume that Mrs. Cosby, comfortable in her life with her marriage contract and children legally binding her to Mr. Cosby's estate, had probably stopped sleeping with him and gone on about her own life separate from his. Another classic marriage-with-money scenario. He constantly gets attention from many younger and very attractive women (the free-sex pitch that is so common around moneyed men) and isn't getting any at home so he eventually succumbs, perhaps awkwardly in this case.
