That's not exactly shady, they're just trying to find out who is financially responsible. If you slipped and fell on someone's broken steps, his liability insurer is responsible. If it's work related, workmen's comp is responsible. Of all the screwed up things health insurance companies do, this is one that makes sense to me.

Yep, standard procedure, and I've dealt with it many times over the years. It is infuriatingly absurd to have some "doctor" hundreds of miles away look at a piece of paper and tell you that the treatment the doctor who examined you prescribed isn't medically necessary. It's the shady way insurance companies have rationed care for decades, but in my experience it got much worse about 10-12 years ago.
