Are you sure that it is 5th and reverse that it won't go into?
Really, REALLY try the shift coupler adjustment again. See if you can adjust it all the way over so that 5th and reverse can engage (i.e. forget about 1st or 3rd). You will know it is in reverse when the pin pushes outward (toward the front of the car). If you can find that plane, then you have a starting point for your adjustment.
I went through the same fun and games recently. The coupler adjustment is very very sensitive.
