I had impingements in both shoulders. At 34 years old I was having trouble driving, couldn't lift anything, and couldn't raise my arms above shoulder level without a cortisone injection every few months. After the 5th or 6th set of cortisone injections my doctor suggested surgery (subacromial decompression). PT did nothing for me--after getting an understanding of the mechanics I don't see how PT could help an impingement. I found an awesome surgeon who did the right shoulder and then the left about two months later.

The relief was nearly immediate. Each shoulder was 90% within a 2 weeks and 100% within a month. I can do "hands up, don't shoot" with the best of 'em now. I may be an outlier but my suggestion is that if you have a shoulder impingement that is causing trouble, get it fixed.
