Seems like that tensioner had to go bad at some point for the teeth to look like they do. I'm a minimalist, I'd replace the sprockets and chain, check the tensioner, put new oil in and new filter on and run it. After a short time I'd drain the oil, pull the filter and have someone with a large pipe cutter cut it open and see if you have any metal. If no metal, new filter, oil and down the road.
