I think that's a hinge made by Schrock (spelling uncertain) I have those in my house as well. Where the problem comes in is the holes that attach it are different on newer hinges. You should be able to get those at Home Depot. If you have trouble finding it let me know and I can get some from a friend in the cabinet business.
Just looked on Amazon. Type in "Blum cabinet hinge" and you'll find one that will work.
